PHYSICAL AFFECTION: She's receptive though she might be a little awkward about it, especially if someone (particularly boys) pat her head, as that was a gesture of affection her older brother used for her, and while she doesn't remember it, it will still feel weird to her emotionally.

Satoko was infected with a rare disease called Hinamizawa Syndrome that induces violent paranoia and hallucinations, and has been taking regular doses of a vaccine to prevent relapsing. She's since been stabilized but moments of extreme emotions can trigger a relapse which will make her violently paranoid and start scratching at her neck and throat, to the point of bleeding.

Higurashi is a very dark canon in and of itself, and Satoko's role in the story can be extremely triggering. Prior to Gou, her story is wrapped up in severe child abuse after her parents died, when she was taken in by her aunt and uncle. She suffers from a considerable amount of psychosis and paranoia due to having succumbed to Hinamizawa Syndrome when she was little and pushing her parents off a cliff in a paranoid haze because she believed they were going to hurt her.

In Gou and Sotsu however, Satoko goes from a clear cut victim to a villain and perpetrator. She gets wrapped up in a timeloop after spending her teenage years rapidly losing hold of all of her friendships and being isolated and abandoned (it's more complicated than that but) at a deeply horrible private school, and starts using her power to toy with her close friends, manipulating them into spiralling through paranoia until they start killing one another. She does this with the goal of breaking the will of the girl that she loves into never leaving Hinamizawa (or her) so that all of them can stay and "be happy together" forever.
